Form Listing Screen

The Form Listing Screen allows you to easily view all of your forms at once, create new forms, edit, copy, or delete your forms anytime you want.

Create a New Form Wizard Screen

The new form Wizard allows you to enter a set of parameters for your form, such as when you'd like to make the form active, how long you'd like the form to run for (great for surveys!) and how you want the form to be sent to you, i.e., through a Microsoft Excel document, e-mail text, etc.

This is also where you can apply your own template to your form to keep the consistent look of your website. Remember, CogniLogic will never advertise on your form.

Form Detail View

The Form Detail View shows a summary of your form with all of it's element types, i.e., radio buttons, text fields, checkboxes, etc. You can easily edit, copy, or delete elements you don't need. This screen also shows you form elements you have designated as 'required' fields for your users to fill out before submitting the form (in red).

Adding Elements (radios, checkboxes, etc.)

Adding your form elements is easy. All you have to do is follow the instructions, which won't allow you to leave out pertinent information for your form. If you know nothing about programming you'll still be able to create a usable form!

After you create your form, all you have to do is choose to activate it and your automatically given a web address to link your other website pages too. The form's web address will even have your domain name. Your form stays active as long as you want it too!
